Name or Change Your Beneficiary

There are two ways to name or update your beneficiary designations:
■ Log in to MAP (Member Account Portal) to enter updated information directly.
- OR -
■ Use the Statewide Defined Benefit Membership Form and the section labeled "Part C - Beneficiary Designation".
